Minnesota law requires a health care directive to be either notarized or signed by two qualifying witnesses. Notarization is the cleaner option because it avoids questions about witness eligibility later.

Who can act as a witness if we go that route?

A witness generally cannot be the named health care agent, and at least one witness must not be a health care provider caring for the signer. If witnesses are needed, we can arrange them when available.

Can you meet at a hospital or hospice?

Yes. Bedside appointments at hospitals, hospices, assisted living, and nursing facilities are routine for us. Facility visits use our facility rate.

How long does the appointment take?

Typically 15–30 minutes, and we don't rush these conversations.

What should we have ready?

The completed but unsigned directive, and a current, unexpired government-issued photo ID for the signer.

Transparent pricing

Every appointment starts at $75.00 and includes up to two signers plus standard-area travel. A scheduling fee may be added to the base price based on when you'd like to meet — scheduling fees apply per appointment (not per document), and only the highest applicable fee is ever charged.

A $50 deposit reserves your appointment and is applied toward your final invoice. The balance is invoiced after the signing is complete.

A $50 non-refundable deposit is required to secure your appointment. This deposit is applied toward the total cost of your scheduled service, with the remaining balance due at the appointment. If you cancel your appointment for any reason, the $50 deposit will be retained to cover scheduling, administrative costs, and reserved appointment time. If Straub Document Group LLC is unable to fulfill your appointment, including being unable to provide requested witnesses, your $50 deposit will be refunded in full. If you need to reschedule, please contact us as soon as possible. Rescheduling is subject to availability and approval by Straub Document Group LLC.

Appointment Scheduling Fee

One scheduling fee is added to each appointment based on the appointment time. Scheduling fees are added to the base price of the selected service and apply per appointment, not per document. Fees are never stacked — only the highest applicable scheduling fee is charged.
